eligible clients located in North Carolina, New York, and South Carolina, subject to

applicable professional licensing laws and regulations.

Telehealth services are provided only when the treating clinician is appropriately

licensed or otherwise legally authorized to provide professional services in the state

where the client is physically located at the time services are provided.

Clients receiving telehealth services may be required to confirm their physical location

at the beginning of each virtual appointment. Clients are responsible for informing their

clinician if they are physically located in a different state or jurisdiction at the time of an

appointment.

Availability of telehealth services may depend on clinician licensure, the client’s physical

location, clinical appropriateness, applicable state laws and regulations, technology

requirements, and other professional considerations.

Telehealth involves the use of electronic communications and technology. Although

reasonable safeguards are used to protect privacy and confidentiality, electronic

communication may involve risks, including interruptions, unauthorized access,

technical difficulties, or other limitations associated with electronic systems.

Practice and treating clinician reserve the right to determine whether telehealth is
